Population Health Strategy and Improvement Division is looking for a Team Leader to support delivery of the Population Health Framework and lead policy to support health and wellbeing in our communities.
Life expectancy in Scotland is stalling and health inequalities are widening, while demand for our health and social care services continues to increase. There is a pressing need to do things differently if we are to improve the lives of the people in Scotland. The Population Health Framework is our new whole system approach to improving population health and reducing health inequalities in Scotland. It focuses on upstream, preventative action to support the building blocks of health.
As team leader for Population Health in Communities you will have oversight of delivery of the Population Health Framework Programme, working with key partners in the NHS, local government and across SG. Within this, your team will also lead on the role of communities in delivering the Framework and on key areas that support population health at local community level, including:
- Developing a new Social Prescribing Framework for Scotland, and developing policy to enhancing the capacity of the community and voluntary sector to support local health and tackle health inequalities; and
- Supporting NHS Boards as ‘Anchor organisations’, to channel more wealth into local communities e.g. through local employment and procurement.
The post is based in the Health Inequalities Unit, which is part of the Population Health Strategy and Improvement Division within Population Health Directorate. The Directorate has a strong focus on both wellbeing and learning & development, and takes a cross-division approach to reducing health inequalities.
Responsibilities
- Lead work to coordinate and oversee a whole system approach to delivery of the Population Health Framework Programme, including working with key partners in COSLA, PHS and across SG.
- Provide support to the Population Health Framework Programme Board and its Chair to steer delivery of the Framework and provide assurance on delivery of actions. This includes supporting the Chair in reporting to overall HSC governance and assurance arrangements on Health & Social Care Reform in the round, of which the PHF is a core component.
- Lead work to Develop a National Social Prescribing Framework for Scotland to support referral links to health and wellbeing support in local communities.
- Work with team members to enhance the role of community organisations and voluntary sector in supporting local population health, including their role in delivery of the Population Health Framework actions.
- Lead a programme of work to support NHS Boards operate as effective anchor institutions, supporting NHS Scotland to play its role in local employment and community wealth building; including delivery of the PHF action to identify opportunities to increase the impact of NHS procurement spend.
- Build and maintain effective relationships with a wide range of stakeholders from Government, NHS, community and voluntary sectors, including senior leaders.
- Lead and develop a high performing team and actively contribute to development of the Unit and the Division. Including line management responsibility for three members of staff.

Working Pattern
Our standard hours are 35 hours per week and we offer a range of flexible working options depending on the needs of the role. From October 2025, the Scottish Government will require staff in hybrid-compatible roles to work in-person 40% of the time, either in an office or other agreed work location.
