About the Role: We’re looking for a self-driven Senior Pharmacy Technician to join our exceptional Care team to manage patients with long-term conditions and medications in a Primary Care setting whilst working from home. This role is on a 3-6 months fixed term contract with the potential to extend or go permanent. At Suvera, we are more than job titles - we are the architects of our future. Your active role will involve conducting comprehensive reviews of patient information and using your sound clinical judgement to identify needs, address concerns and ensure they receive safe, effective and valuable care. What you\'ll be doing : The position is fixed term, full time (40 hours over 5 days a week) opportunity. To apply, click the ‘apply for this job’ button below or fill out the form at the bottom of this page Accurately recording and cording of patients health data on Suvera Dashboard & Practice EHR, ensuring all necessary reviews/assessments are completed Interpretation, Risk stratification, and Management of this data in line with standard operating procedure Conduct comprehensive reviews of patient information using professional and clinical judgment to identify needs, address concerns, and ensure safe and effective care, delivering lifestyle advice and referring patients to clinicians or other services as appropriate. Including but not limited to the following conditions: Non-diabetic Hypergycaemia (NDH), Hypertension, Asthma, Hypercholesterolemia Conduct structured medication reviews to ensure safe, effective, and appropriate use of medicines, identifying opportunities for optimisation, communicating need for change to Clinical Pharmacists Reviewing clinical letters and completing medicine reconciliation Gain informed consent for medication changes, provide patient education and counselling on prescribed medicines, including advice on adherence and managing side effects. Ensuring appropriate tests are ordered and completed by patients. Identifying and resolving medicine related queries, ensuring safe prescribing practice and adherence to NICE guidelines. Complete clinical tasks independently and asynchronously, ensuring thorough assessment and appropriate action, while escalating complex cases or concerns to the relevant clinician when required. Process medicine requests, ensuring accuracy, appropriate monitoring, and alignment with formulary and prescribing guidelines. Support governance and patient safety efforts by participating in quality management processes and auditing of your calls. Maintaining at all times a regard for patient privacy, safety and dignity Supporting and participating in shared learning within Suvera and our GP Partners Use your expertise to help us shape the development and improvement of the Suvera’s systems We\'re looking for someone who: Should have: Currently working as a Registered Pharmacy Technician within NHS and have more than 12 months experience in primary care Completed a two-year accredited pharmacy technician course involving a mix of practical work experience and study via GPhC-accredited courses, GPhC-recognised qualifications or a GPhC approved apprenticeship pathways Completed your CPPE pathway Evidence of continuing professional development (CPD) relevant to the role Hold a recognised Medicines Optimisation Qualification e.g MOPS Experience communicating with patients via multiple channels, including email, messaging, telephone or video calls Demonstrable communication skills, written and spoken Tech savvy and IT literate, you must be able to operate multiple software systems seamlessly and simultaneously. This includes, EMIS, SystmOne, Slack and Microsoft Office.
