This position provides specialized knowledge in substance abuse, domestic abuse, grief counseling, workplace problems, and management consultation.<br /><br /><b>CORE JOB FUNCTIONS</b><br />1. Advocates for and helps clients gain access to resources while confronting their personal issues.<br />2. Counsels and provides crisis intervention for patients, including assessment and treatment of behavioral problems.<br />3. Coaches patients on how to manage ongoing health conditions.<br />4. Provides therapy to help patients make positive behavior changes.<br />5. Assists patients and their families through individual, group, and education conferences to understand, accept, and follow treatment recommendations.<br />6. Evaluates the effectiveness of counseling programs and clients’ progress in resolving identified problems and moving toward defined objectives.<br />7. Refers patients and their families to specialized treatment programs in accordance with their needs, and monitors and evaluates their progress.<br />8. Collaborates with other departments to integrate care where needed.<br />9. Maintains current and appropriate documentation in patient chart.<br />10. Maintains confidentiality of records relating to clients’ treatment.<br />11. Acts as a mandated reporter in suspected child abuse cases.<br />12.
