Job Summary

WellBeing Nigeria is seeking compassionate and dedicated Community Health Workers (CHWs) to serve as vital links between local communities and healthcare services. As a CHW, you will provide preventive, promotive, and basic healthcare support, educate community members on health practices, and assist in improving overall community wellness. This role is critical in bridging gaps in healthcare access, especially in underserved areas.

Key Responsibilities

Conduct home visits and community outreach to identify health needs and provide support.
Educate community members on hygiene, nutrition, maternal and child health, disease prevention, and healthy lifestyle practices.
Assist in monitoring chronic conditions, vaccination drives, and other public health programs.
Serve as a liaison between healthcare providers, government agencies, and the local population.
Maintain accurate records of health interventions, patient follow-ups, and community health reports.
Participate in health promotion campaigns and community events.
Identify and refer cases requiring medical attention to healthcare facilities.
