They are responsible for providing evidenced-based care for the management of patients, including assessment, implementation and evaluation of a detailed plan of care, which incorporates diagnostic and therapeutic interventions. The APP demonstrates competency in providing age-appropriate care to patients as defined by certification and training and provides family centered care to meet family needs, including psychological support, education and discharge planning in conjunction with other related disciplines.<br><br>The APP initiates, participates and collaborates with divisional and organizational multi-disciplinary staff to provide and support evidence-based education, quality improvement and research. They participate in professional and organizational activities that contribute to improving patient care, the scientific knowledge base of advanced practice, program operation, and individual professional development. The APP provides leadership within their respective clinical area through participation in division projects and mentorship of others.
